It's not so easy to do, but if you're willing to put in the time and effort, what you will need is an engine and some development tools, to make models, textures etc.
If you want to spend money you can get a 1st-class game engine and devkit from Garage Games, it's called Torque. The commercial version is used to develop a lot of games sold today. The version for individuals is $100.
If you aren't planning on designing the next Quake though, you could try some of the better open-source or gpl engines. Crystal Space is one, irrlamb is another. Google the names to find them.
The easiest to use though, and the easiest to learn, is the Cube engine. The latest fully developed version is called Sauerbraten, also called Cube 2. With it you can design multiplayer maps including in capture (zones, or bases) mode, and you can make singleplayer levels as well. There are tons of built-in textures and models, and you can make your own too. You can set triggers to open doors, make animated pools of water, put different weapons and armor here and there, put in an end-of-level objective, all kinds of things. It actually is being used by a development company to make a commercial game too.
Besides that, it is a game in its own right, plenty of maps there already and online multiplayer too. You can stop the game any time, go into edit mode, and edit the terrain. (it's considered cheating in actual games though.)
